RESOLUTION AUTHORIZING RENEWAL CONTRACTS FOR
EMPLOYEE GROUP HEALTH INSURANCE PLANS WITH EMPIRE BLUE
CROSS BLUE SHIELD

WHEREAS, the Town of Queensbury previously entered into agreements for employee group
health insurance plans with Empire Blue Cross Blue Shield, and
WHEREAS, the Town Controller has negotiated renewal contracts for the year 2000 with
Empire Blue Cross Blue Shield and has presented the contract rate adjustment pages delineating the
proposed 2000 monthly premiums at this meeting, and
WHEREAS, the proposed contract rate adjustment pages have been presented at this meeting
and are in form approved by Town Counsel,
NOW, THEREFORE, BE IT
RESOLVED, that the Town Board of the Town of Queensbury hereby approves and
authorizes renewal contracts for the year 2000 with Empire Blue Cross Blue Shield as presented in the
contract rate adjustment pages presented at this meeting, with the 2000 month premiums as follows:
