RESOLUTION AUTHORIZING ADOPTION OF GROUP HEALTH
INSURANCE COST SHARING FORMULAE FOR ELIGIBLE RETIREES

WHO MOVED ITS ADOPTION

WHEREAS, eligible retired Town of Queensbury employees contribute toward the premium
and receive group health insurance and prescription drug coverage provided under the Empire Blue
Cross Blue Shield Matrix Plan, Empire Deluxe PPO Plan or the Blue Choice HMO plan, and
WHEREAS, the Town Board of the Town of Queensbury wishes to adopt group health
insurance cost sharing formulae for all eligible retirees effective January 1, 2000,
NOW, THEREFORE, BE IT
RESOLVED, that the Town Board of the Town of Queensbury hereby adopts group health
insurance cost sharing formulae for all eligible retirees effective January 1, 2000 as follows:
Retiree Town Medicare Premium
Share Share Reimbursement
Matrix 15% 85% 85%
Empire Deluxe PPO 9% 91% 91%
Blue Choice HMO 0% 100% 100%
and
BE IT FURTHER,
RESOLVED, that the Town Board hereby authorizes and directs the Town Controller’s Office
to sign any documentation and take any and all steps necessary to implement the terms of this
Resolution.
