Dear Dave:
In an effort to provide the Town with final compliance documentation regarding the wastewater
system serving the referenced property I offer the following.
The design plans, prepared by this office and reviewed and approved by the Town as associated
with the wastewater system application,are as follows:

During the course of construction I was onsite on numerous days observing the installation of all
major components of the system. It is noted and represented by the attached "As-Built' plan that
several minor changes were made to the system during construction. Those include a slight change
to the septic tank and reactor tank locations and the sizing of each tank. The tank capacities remain
the same ....Septic—2000 gallons and Reactor—1000 gallons. The enhanced treatment
mechanisms (PuraFlo Sequential Batch Reactor)were installed, programmed and set into operation
in May 2020. 1 have monitored its operation and the wastewater system has operated successfully
and as designed over these past five months. It is my opinion that the wastewater system has been
installed in compliance with the approved plans and is operating as intended.
