The application for each mobile home permit shall, be in writing, signed
by the ~pplieant and shall beaceompaned by a plan drawn reasonably
to scale and showing the boundaries of the premises. The location and:
plan of the proposed water supply and sewage disposal system. If the
'applicant is not the. owner in fee of the premises then applicationmust
be accompani®d by a certified or photostatic copy 'of the written
lease, or if there be no written lease,'by the written `acknowledged
