This location is not located within a Town of Queensbury designated CEA.
These tests are for a proposed replacement of an existing failed system The proposed system is to be installed
in the same general area as the deep holes and percolation test holes .
The bottom of the new system will be approximately 24" below existing grade. The soils have an average
stabilized percolation rate of 8.4 minutes.
• A review of the system plans by Scudder Engineering, dated December 27, 2006, is consistent
with the information listed above as well as the requirements of both the Town of Queensbury
and appendix 75-A of the New York State Health Department regulations.
Because this is a replacement of an existing system it is not mandatory that a 50% expansion area be
designated. If an expansion or replacement is necessary, a new system will have to be designed.
Attached is a copy of the inspection sheet indicating the information witnessed during deep hole and
percolation testing. Please contact me should you have arty questions relating to this project.

Submitted herewith is a plan for improvements to the failed on-site wastewater
system at the Gallucci residence.
I.B.S. Septic & Drain, Inc. will be at the Gallucci home at 3P.M, today prepared to
dig a deep test hole and do a subsurface investigation as to the cause of failure
of the existing system.
Ralph Van Dusen affirms that he will be available on the site at 3:30 P.M. today
to witness the deep test hole and percolation test. We ask that you please
contact Mr. Van Dusen and authorize the coordinated team to proceed as above
this afternoon at 3:00 P.M.
