Dear Mr. Schermerhorn:
During a site plan inspection of your project, the following deficiencies were
discovered:
1) No evidence of any rip-rap at outfalls in north detention basin.
2) No evidence of any rip-rap at outfalls in southwest detention basin.

3) No evidence of any rip-rap at north end of outfall in southeast detention cm
basin.
4) Smaller drain line installed (not proposed or approved) at lower elevation
than that of approved culvert from southeast detention basin (drains into
Old Maid Brook).
5) Large silt/sediment bed formed in area just before entry into east culvert
and into east culvert of Old Maid Brook under Willow Brook Road.
Additionally, silt deposited just downstream of the culvert.
�J �6) Erosion gully formed at north end of north grass swale resulting in
silt/sediment deposition into wetland at north end of lot.
7) Well used to provide water for irrigation of landscaping next to stream not
,proposed or approved (is water being pumped from Old Maid Brook?).

9) 7 curbed islands designated as "lawn" filled with cobble size "river
rocks".
10) Areas designated to be "lawn" on east side of building filled with cobble
size "river rock".
