Dear Mr. Schermerhorn:
During a site plan inspection of your project, the following deficiencies were

On 12/01/03, the southeast and southwest detention basins (deficiencies 2
and 3) had rip-rap added to their outfalls; none was observed added in the
� north detention basin (deficiency 1)t�:
L 4) Smaller drain line installed (not proposed or approved) at Cower elevation
than that of approved culvert from southeast detention basin (drains into
Old Maid Brook).
�5) Large silt/sediment bed formed in area just before entry into east culvert
and into east culvert of Old Maid Brook under Willow Brook Road. ,_, C,K ) I'JJ41
Additionally, silt deoposited just downstream of the culvert. � J
6 ;Erosion gully formed at north end of north grass swale resulting in
silt/sediment deposition into wetland at north end of lot.
09/25/03, the erosion was found filled with river rock
r --�>7) Well used to provide water for irrigation of landscaping next to stream not
t proposed or approved (is water being pumped from Old Maid Brook?).
�8) 7 curbed islands designated as "lawn" filled with cobble size "river
rocks".
9) Areas designated to be "lawn" on east side of building filled with cobble
size "river rock".

�- 0) Two existing large spruce designated as "to remain" were removed in
violation of approval (large one at dbh = 24" to 28" and smaller one at 18" to
40"). (see photos attached)
11) 4 flowering crabapple trees by sign at corner missing.
(still not done on 12/01/03)
12) One Summit Ash missing along Willow Brook Road (next to southeast
detention basin).
j 13) 25 Stella D'Oro Daylilies missing in front of main entrance (curbed
Zand filled with concrete). (Landscape contractor, Rob Wing claims the 25
plants were planted along the building instead of where approved.)
14) Filter fabric needs to be removed from catch basin grates (CB1 and
CB4).
