Provide area protection as necessary for existing walls, flooring and floor coverings. Remove electrical fixtures and audio components and save for reuse, with subcontract removal of alarm components to NY Fire & Signal. Remove suspended ceiling systems and remove 1900 +/- SF loose laid insulation above, storing insulation for reinstallation. Remove fiberboard and batten ceiling above insulation in main sanctuary while retaining original lath and plaster ceiling. Remove the nonstructural box beam at east edge of the chancel area and remove the extraneous 7"+/- frame down and sheetrock at the bottom of the E-W main beam chase. Dispose of all Debris Install new 11 7/8" TH 230 ceiling joist system in chancel area with N/S joist 28'+/- @24"OC and either 7/16" OSB floor over, or TH cross blocking @ 48 OC, to prevent roll. Fabricate and install perimeter ledgers and wood frame downs in main sanctuary area as per plan and reframe bottom of chase at E-W main beam to allow sheetrock cover to be installed tight to bottom of girders to minimize downward intrusion. Relocate electrical rough to plan and arrange for electrical inspections. Repair the top section of central column chase as needed, sheetrock the bottom of the E-W main beam chase, infill repair perimeter walls as needed to new ceiling height and install 5/8"Type X sheetrock ceiling throughout. Tape and compound ceiling, chases and wall repairs to paint grade. Prime and paint all new sheetrock surfaces and reinstall electrical fixtures and audio to plan. Install removed R19 insulation, and additional new insulation as needed, to provide an insulation cap of 2 layers of R19, loose cross laid above the original lath and plaster ceiling and the newly framed chancel section. For new insulation materials a $250.00 allowance will be used to supply approximately 750.Sq ft of new R19 insulation to augment the existing insulation removed. Additional insulation required over the allowance, due to any of the existing material removed being not suitable for reuse, shall be reimbursed at cost+ 15%. Remove all area protection, clean wall and floor surfaces, and complete replacement and testing of alarm components by NY Fire & Signal. The Ceiling Electrical Plan is not yet completed as a reorientation of the sanctuary is under consideration at this time. The Construction Budget submitted shall exclude electrical subcontract costs but shall include the supervision and scheduling of the electrical subcontractor. A final electrical plan will be submitted for bid prior to awarding of the project contract. The Bay Road Presbyterian Church will provide normal heat, electricity, water and bathroom facilities on site. The contractor shall provide any supplemental heating, store all materials and debris in designated areas, and maintain the work site in an orderly condition. The General Contractor is responsible for the entire project including all scheduling, the supervision of all subcontractors, and obtaining all inspections required. The Bay Road Church is responsible for any permits. The General Contractor shall deliver with their bid an estimate of approximate length of project completion. The entire sanctuary, south and east entrances, and conference room shall be exclusively available to the Contractor as shop/work/secure storage space, and the Bay Road Church will not utilize those areas, for the duration of the project. The General Contractor shall deliver certificates of insurance for liability and workers compensation before work begins. During that project a detail was changed so we asked Mr. O'Brien if an addendum drawing for that change should be filed. He informed us that would not be necessary because Queensbury requires commercial buildings,which is Bay Road Church's classification,to have all framing inspected and certified to plan,not only by the Town Inspector,but by the Architect of Record as well,before it is covered by sheetrock. Therefore he said the architect could simply"sign off'on that detail at the end of the job. We then discussed that the most critical aspect in proper performance of the new ceiling framing was the installation of the 6" Timberlock fasteners securing the top plate of the new box beams to the 6"X6" bottom member of the original Queen Trusses that serve as the current ceiling joists. These fasteners will be concealed in the box beams when they are completed,long before sheet rock,so the proper installation cannot be confirmed after the box beams are in place,but must be observed as the box beams are being fabricated and installed.. This amount of oversight would have been extremely difficult to ask the Architect of Record to accomplish, given that he lives and works in Troy, is a member of the Presbyterian Church there and had already provided both the engineering and design work as a donation to the Bay Road Church. We then asked Mr. O'Brien if it would be acceptable for us to take photographs of the open system before the bottom plates were installed, and provide the architect with those photos so the architect could then confirm that proper completion. Mr. O'Brien said that would be entirely up to the architect,as to how it was accomplished, as long as the architect would "sign off' on the work. We then discussed this solution with the architect by email and by phone on 2/21/13 and he agreed to that solution, so we continued under that premise. The Bay Road Church sanctuary ceiling project was successfully completed, including the painting,for Palm Sunday service in 2013. Mr. O'Brien has requested,several times,that we supply the Queensbury Building Department with appropriate documentation to properly close out this permit. To date,however, our attempts to obtain some sort of signoff from the Architect of Record have not been successful,so we have discussed alternatives to fulfilling this requirement with Mr.O'Brien, In Westchester County,where I owned and operated a construction firm for over 35 years, a Building Permit was closed with a formal aaffidavit called a Certificate of Construction Compliance. This legal document, sworn to by the Architect or Engineer of Record,or a qualified individual who had overseen the construction, stating that the permitted construction had been completed as per the approved plans. This did not replace Final Inspection by the Code Enforcement Officer, but confirmed that closed or covered elements of the project had been properly completed in accordance to those approved plan,with any exceptions noted. As I held a Westchester County Contractors License,and had over 10 years of experience in my field,I was authorized to file an Affidavit of Final Construction.
