Please review the requirements in Sections 3 and 4 carefully. Compliance with this permit is required by law. Failure"to perform the septic tank pump outs and inspections required in Section 3 can result in the revocation of this permit, the loss of the privilege to use your wastewater system and, ultimately, the loss of the use of your house. This permit is issued in accordance with the Lake George Park Law. The purpose of the program is threefold. First, to insure that failed systems are identified and promptly corrected through the cooperation of system owners using the information gathered during regular inspections. Secondly, to work cooperatively with system owners to provide for the upgrading, over time, of substandard systems, individually or cooperatively among several system owners working in common. Thirdly, to emphasize regular maintenance as an important aspect of proper system performance. All systems ultimately fail and must be replaced. However, regular pump outs and the replacement of worn components can greatly increase not only the performance but.the useful life of the system. SECTION 2: Basis for determination of permit type 1. X Because the wastewater treatment system meets the minimum requirements for,existing systems pursuant to Section 646-3.6, the system is determined to Meet Existing Standards and a five year operating permit is being issued. The permit requires regular maintenance to ensure good performance. 2. ' Because the wastewater treatment system meets the design standards for new systems pursuant to Sections 646-3.6 and 646-3.12, the system is determined to Meet Design Standards and a five year operating permit is being issued. The permit requires regular maintenance to ensure good performance. SEt TION 3: Permit Requirements 1. Any expansion in size or increase in use of the buildings or wastewater generating fixtures connected to the wastewater treatment system shall require prior approval by the Town and may require a • permit for construction of a new*or modified wastewater treatment system. 2. Any failures or breakdown of any component of the wastewater treatment system or any flow of wastewater onto the ground or into the waters of the Park must be reported immediately to the Town. Remediation must be completed within 24 hours of the break or failure. • 3. All components of the wastewater treatment system must be:maintained in good working condition in accordance with manufacturers' recommendations as well as special conditions contained in this permit (if any). - 4. Bath septic tank on the property must be pumped out within the next 24 months and inspected every two years thereafter to determine scum and sludge accumulation. Scum and sludge shall be measured in the first compartment.of a two compartment tank, or in the first tank of a multiple tank system. After the initial pump out, septic tanks shall be pumped when the bottom of the scum layer is within three inches of the bottom of the outlet baffle or tee, or when the sludge level exceeds 1/3 the liquid depth.of the tank. Enclosed is a certification which must be completed by the licensed waste hauler at the initial pump out. This certification must be signed by the licensed waste hauler and yourself and returned to the Town (see instructions on the back of the form). SECTION 4: Special Conditions 1. This permit and the authorization to discharge shall expire on midnight of the expiration date shown and the permittee shall not discharge after the .expiration date unless this permit has been renewed or extended pursuant to law: To be authorized to discharge beyond the expiration date, the permittee shall apply for a permit renewal on the application forms provided by the Town no more than 60 days after receipt of such forms.
