OFFICERY 169 Haviland Road Queensbury,NY 12804 Re: Onsite Wastewater Treatment System(OWTS) and Public Water System(PWS) Proposed Charlie's BBQ (Hoertkorn Property) (T) Queensbury,Warren County Dear Mr.Nace, Enclosed please find three stamped sets of the approved OWTS and PWS design plans, last revised on October 13, 2004, for the above referenced facility. Please provide one copy of the plans to the facility owner and one copy to the(T) Queensbury Code Enforcement Office. One set of the approved plans is being retained for our files. The OWTS design consists of a 1,500-gallon grease trap, a 2,500- gallon septic tank, a pump station with a duplex pumping system and 600-feet of absorption trenches. The PWS consists of: a new drilled well, a submersible pump, a 40-gallon hydro-pneumatic pressure tank, a water meter, a 5-micron filter, an ANSI/NSF Standard 055 Class A Ultraviolet(UV) Light Disinfection Unit with an automatic solenoid shut-off valve and raw and finished water sampling taps. Plans also include a line for a possible sprinkler system that shall be equipped with a backflow preventer. This approval is conditional on the following: 1. That the proposed OWTS and PWS.are constructed in accordance with the approved plans. 2. That a design professional licensed in New York certifies that construction was completed in accordance with the approved plans. 3. That the construction certification is submitted to the(T) Queensbury Code Enforcement Office and to the NYSDOH Glens Falls District Office. 4. That a spare ultraviolet tube and any other equipment necessary for prompt repair of the UV disinfection unit is provided on site. 5. That the proposed PWS not be placed into operation until one sample that tests negative for Total Coliform has been collected from the UV system, and the results have been submitted to and acknowledged by the Glens Falls District Office. 6. That one additional sample of treated water is collected for Total Coliform analysis and the results submitted to the Glens Falls District Office within the first month of operation of the new system. 7. That this approval will expire on October 19, 2006. I cannot complete my review without the following information: • There was no submission for the UV disinfection included in the packet. Please have your installer submit the necessary samples and documentation needed to review this system. I have once again included the necessary paperwork for this submission. • There were no details in the building including details of the bathroom layout. It would appear that the public toilets have doors on the outside. • There were no lists of equipment specifications, only for two specific items. I need documentation on all food service equipment. • The wall under the hood must be stainless steel. We have absolutely nothing in the way of drawings or information showing the current configuration venting through the combustible wall. My research indicates several items that appear to be in violation of NFPA-96,as follows: Section 4-2 requires 18"clearances to combustible construction for the duct. If the required clearances cannot be met,an approved clearance reduction system is required. Section 4-3 requires at least one 20"x20"clean-out opening. I did not see any openings in the duct. Section 4-5.2.1 requires liquid-tight continuos external welds at all seams.What we observed did not appear to meet this standard. Section 4-8.3 requires 10 feet of clearance to combustible construction and operable doors for through-the-wall duct vents. The wall itself is combustible.
