DECLARATION:
1. 1 acknowledge that no construction shall be commenced prior to the issuance of a valid permit and will be
completed within a 12 month period.Any changes to the approved plans prior to/during construction will require
the submittal of amended plans,additional reviews and re-approval,
2. If,for any reason,the building permit application is withdrawn,30%of the fee is retained by the Town of
Queensbury.After 1 year from the initial application date, 100%of the fee is retained.
3. Ifthework is not completed by the 1 year expiration date the permit may be renewed, subject to fees
and department approval.
4. 1 certify that the application,plans and supporting materials are a true and a complete statement and/or description
of the work proposed,that all work will be performed inaccordance with the NYS Building Codes, local building laws
and ordinances, and in conformance with local zoning regulations,
5. 1 acknowledge that prior to, occupying the facilities proposed I, or my agents, will obtain a certificate of
occupancy.
6. 1 also understand that I/we are required to provide an as-built survey by a licensed land surveyor of all newly
constructed facilities prior to issuance of a certificate of occupancy.

THE WORK TO CONSIST OF:
Three Season Room:Size 12'by 12':Color is white:Style is Elite Design : Free insulated glass.Gable slope 3"roof
1.Customer to take up existing deck boards,and railings and haul away.Patio to Double box perimeter with 2"by 8", add joist
hangers where needed, lag the.deck more secure to the house.Install.5 engineered techno post footings. Install a new 3/4"tongue
and grooved advanteck subfioor.Install solid vinyl skirting around the deck perimeter below the glass.
2. Manufacture and install a.3"r-15 foam insulated,gable roof,thermally broken using structural I beams to support it. Install 2 glass
gable wings, and 2 gutter-and downspouts.
3. Manufacture and install(4)-66"tall rolling glass and screen windows on a 18"glass kneewall,and 2-84"tall rolling glass and
screen doors.All glass 5/8" insulated using soft-coated law/e,argon gas,and foam spacers.
4.Install'3 electric.posts,with 3 duplex outlets,install customer provided ceiling fan, and coach light. Install switches for fixtures. Run
a seperate line out from customer existing box, and obtain electric permit.
5. Patio to create NY State Stamped engineered drawings, site specific to customers area and snow load. Permit obtained by Patio.
No gurantees on pre-existing house conditions.
