This is to certify that the Asbestos Inspection Survey for.the above site location has been conducted in accordance with
the requirements of the New York- State Attorney General Asbestos Inspection Guidelines, The New York State
Department of Labor and The New York State Department of Health.
Objective
The objective of this inspection was to determine the presence of Asbestos Contained Building Material in suspect bulk
samples. If ACBM's are detected they are then identified and quantified into groups (Serpentine or Amphibole) and
then broken down to classes. The classes include Chrysotile, Amosite, Anthophlite, Crocidolite, Antinolite and
Tremolite.
Site Inspection and Material Analysis
The above structure being primarily of concrete and wood framing was physically and thoroughly inspected for
asbestos laden materials per the requirements of code rule 56-1.9. This.includes surface treatments, thermal system
insulation, roofing and siding, other material and all subparts thereof. Bulk samples of all suspected,materials were
collected for testing on the basis of my professional knowledge with building-materials and known areas of usage for
asbestos materials.
All suspect materials are studied under plane-polarized light and orthoscopic illumination to determine refractive indices
along specific crystallographic axes and antisotropism. These studies involve immersing the sample in a know refractive index liquids and qualitatively determining the mineral types by refractive index, morphology, signs of
elongation, birefringence characteristics and extinction angles. The correlating mineral type is then ascertained.
A petrographic point counting technique is then applied to the sample under investigation to quantitative outline a
percent composition of mineral fiber in the matrix of the bulk sample.,- This counting involves representing sections of
the entire material if it is homogeneous. If the entire material under study is heterogeneous, each strata of the sample
that can be isolated as a homogeneous section and would be considered and treated separately.
Asbestos Inspection Results
It has been determined from both the on site inspection and the laboratory analysis of the samples that the following
has tested positive for asbestos:
The complete kitchen floor covering approximately 120-sq. ft. is a white, in good condition, homogeneous and non-
friable.
