STORMWATER POLLUTION PREVENTION PLAN MODIFICATION REQUEST
This form is to be used only when the consultant believes modifications are needed on
an existing SWPPP and/or Erosion and Sediment control plans. For example,
additional erosion control measures are needed or removal of specific control measures
can be done without adverse impact. This form must be approved by the Town
Stormwater Management Representative prior to implementation.

CHANGES REQUIRED TO THE STORMWATER POLLUTION PREVENTION PLAN: Changes to the
Stormwater Pollution Prevention Plan include a revised erosion and sediment control plan, and stormwater design
based on the additional 4.03 acres to be added from a lot line adjustment with the adjacent parcel. Approximately
3.3 acres of gravel storage and laydown area will be added to the project site.
REASONS FOR CHANGES: The original Stormwater Pollution and Prevention Plan (SWPPP) was designed for
the existing project site not including this added land and impervious area. We are requesting to make the
aforementioned modifications to the SWPPP to maintain erosion and sediment control in accordance with the New
York State Stormwater Management Design Manual and New York State Standards and Specifications for Erosion
and Sediment Control.
