Principal Me6hanical Designer-Name Signatur Da MEMO TO FILE RE: SCHERMERHORN MEDICAL OFFICE BUILDING BAY ROAD MARCH 25,2003 On March 24, 2003, at approximately noon, I met with Rich Schermerhorn, Frank Dingman, from Electric, Donald Loveland from Commonwealth Inspection Agency to go over the requirement for an additional ground to be placed in the medical treatment rooms at the medical office building. Previous to this date, Mr. Loveland had stated that the MC Cable met the requirements of Code Section 517.13 of the National Electrical Code. Further review and consultation with electrical suppliers revealed that the electrical manufacturers will not state in writing that their MC Cable meets this requirement of Code Section 517.13 because they also manufacture a cable that has a fourth conductor for grounding that meets the requirements for hospital grade insulation. While there was disagreement as to whether the MC Cable provided in this building would meet this requirement, Electric has agreed to add a grounding wire to all the outlets and switches in the rooms pertaining to patient treatment. They would be running this ground wire through the boxes and the boxes would be installed with a hospital grade electrical outlet which would be visible at time of inspection. During this meeting, we identified all the rooms in a purple marker that were required to meet this requirement. It was also understood that Don Loveland will re-inspect this building to ensure that this grounding wire has been properly installed. This Department will also inspect to make sure that a grounding wire has been installed in the building, although we are not inspecting it for compliance, other than to verify that the wire is in the building and home run back to a panel box for grounding purposes. TOWN OF QUEENSBURY 742 Bay Road, Queensbury, NY 12804-5902 518-761-8201 March 7,2003 Rich Schermerhorn 15F Birdie Drive Queensbury,NY 12804 Dear Rich: This letter is in reference to our conversation regarding the installation of a sprinkler system and work related to the ceiling of the medical office building currently under construction on Bay Road. This-letter.will serve as a STOP WORK ORDER that no furthei work on the sprinkler system or sheet rocking of the ceiling first floor can commence until such time as this.office has received a sprinkler plan approved and accepted by the Fire Marshal's office for installation Once that plan has been received and approved, this Department will go over what requirements the contractor must meet for, installation of the sprinkler system. This office will not allow the ceiling of the first floor to be sheet rocked and sprinkler heads installed through the sheet rock into the truss space. The piping must be installed prior to sheet rocking of the ceiling for inspection by the Fire-Marshal's office to ensure that all clearances to the sprinkler heads have been met. Also, a revised drawing needs to be submitted to show the changes in the access area to the pool basement. As always,if you have any questions, you may contact me at any time or Steve Smith, the Fire Marshal. Thank you for your attention in this matter.
