The application provided important information on the proposed project. As listed below, initial evaluation by Agency staff indicates that additional information is necessary to review the project and complete the application. You will receive a notice in writing informing you when staff has received the information necessary to complete the application. At the time the application is deemed complete, the required time period for Agency action on the proposed project will begin. The proposal may not be undertaken until a permit has been issued by the Agency. “Undertake” means any commencement of a material disturbance of land preparatory to the proposed project, including but not limited to road construction, grading, installation of utilities, excavation, clearing of building sites, or other landscaping, or in the case of subdivision, the conveyance of any lots. Electronic copies of plans must be fully scalable. 1. Application For Minor Project Permit (Application) Item 5 – Site Plan: The provided site plan indicates that an existing woods road crosses wetlands and will be utilized for access to the proposed single family dwelling. Please clarify if any grading or resurfacing is proposed for the existing woods road. If improvements to the existing woods road are proposed, an erosion and sediment control plan must be provided within 100 feet of wetlands, including at a minimum silt fence or a similar erosion and sediment control down gradient of the proposed area of disturbance and upgradient of wetlands. Please also indicate the existing and proposed limits of clearing on the updated plan. 2. Application Item 9 – On-Site Wastewater Treatment System: Agency Q-4 standards require a minimum vertical separation of 24” between trench bottoms and evidence of seasonal high groundwater, and a minimum of 48” between trench bottoms and evidence of bedrock or machine refusal. To meet Agency Q- 4 standards please revise the provided plans to specify that these standards will be met, which will require a maximum trench burial of 11” below native grade for the provided data. Or alternatively, please provide data for an additional test pit to confirm absence of bedrock or machine refusal to a minimum depth of 67” with a corresponding proposed maximum trench burial at 19.” 3. Application Item 11 – Historic Preservation Resources: The project site is located within an archeologically sensitive area according to the NYS Cultural Resource Information System (CRIS) online map. The project site is also improved by structures that are greater than 50 years old. A submission to the New York State Office of Parks, Recreation and Historic Preservation (OPRHP) will be necessary as part of the consultation process required by the State Historic Preservation Act. Please be advised that the Agency cannot deem an application complete until the OPRHP’s determination and/or recommendations for historic resource impact mitigation have been provided to the Agency. 4. Application Item 14 – Other Governmental Reviews: Please have the Local Government Notice Form completed, signed by the appropriate town official and submitted to the Agency.
