WHEREAS, by Resolution No.: 308,2022, the Queensbury Town Board authorized
the Town Water Superintendent's engagement of Siewert Equipment (Siewert) to refurbish
a Vertical Turbine Pump (Pump) at the Town's Water Treatment Plant in accordance with
Siewert Equipment's quote/repair proposal dated July 28, 2022 for the amount of$20,572,
and
WHEREAS, the Water Superintendent has advised that Siewert's inspection report
of the Pump documented that the wear and tear on the Pump was more than the "normal"
expected wear and tear and that in addition to replacing the expected wear parts of the
Pump, the line shaft and couplers should also be replaced, and
WHEREAS, such additional repair work resulted in Siewert's quote increasing
from $20,572 to $28,310 as set forth in Siewert's October 21, 2022 quote presented at
this meeting, and
WHEREAS, the Water Superintendent has recommended that the Town Board
authorize Siewert's revised quote for such necessary, important repair work,
NOW, THEREFORE, BE IT
RESOLVED, that the Queensbury Town Board hereby authorizes the Town Water
Superintendent's engagement of Siewert Equipment to refurbish a Vertical Turbine Pump at
the Town's Water Treatment Plant in accordance with Siewert Equipment's revised
quote/repair proposal dated October 21, 2022 substantially in the form presented at this
meeting for the revised amount of$28,310 to be paid from Account No.: 040-8320-4350,
and
BE IT FURTHER,
RESOLVED, that the Town Board hereby authorizes the Town Budget Officer to
take all actions necessary to amend the Town Budget and effectuate the transfer of$7,738
from the Water Department's Undesignated, Unassigned Fund Balance Account No.:
040-0000-0909 to Account No.: 040-8320-4350, and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs the Town
Supervisor and/or Town Water Superintendent to sign the repair proposal and/or any other
needed documentation and the Town Supervisor, Town Water Superintendent and/or
Budget Officer to take such other and further actions as may be necessary to effectuate the
terms of this Resolution.

Customer initially reported that pump has decreased in efficiency and requests a DCIIRepair Proposal including a SECO
Technician to help with re-installation of the pump and motor,setting impeller clearances and coupling,and also
installing the mechanical sea[.The inspection has been complete as per inspection report provided separately,The
sight-unseen repair estimate was updated to include the additional parts and services determined to be required and
provided to the customer.The repair proposal as been updated to reflect the customer opting to replace only the
originally quoted parts and the parts listed below.

NOTE: Customer remains responsible for removing the pump from the system to include rigging/craning,transportation
to and from Siewert Equipment's Albany Shop,riggingtcraninglre-installation of the pump back into the system,and re-
wiring the motor.Proposal includes one Siewert service technican to assist&provide guidance during reinstallation of
the pump and motor,setting impeller clearances and coupling,and also installing the mechanical seal,limited to one
normal work day.
1.)Provide parts and labor required to rebuild aforementioned pump utilizing the attached parts list.Genuine OEM
replacement parts will be provided unless specifically noted otherwise,
2.) If Siewert Equipment Company determines that additional parts/labor are required that were not included in this initial
quote,you will be notified for review and approval before we proceed.
3.)Should customer choose to not proceed with the repair work proposed,an inspection fee will be charged in
accordance with the costs associated with product disassembly,clean,and repair assessment,
4.)Upon receipt of inspection findings and repair assessment,customer is responsible to advise whether to proceed
with equipment repair,return unrepaired,or scrap.Storage is free for up to 30-d*,and chargeable at a rate of$10 per
day beyond 30•days.Please be advised inspected equipment may be scrapped by Siewert Equipment if not notified
otherwise within 60 days.
5.)Payment terms are Net 30 days.A 4%processing surcharge applies to all credit card orders.Merchant name on
credit card statement will be Cummins-Wagner.
This proposal is valid for 10 business days from date on proposal and is subject to attached terms and conditions
