WHEREAS, the Queensbury Town Board wishes to enter into an Agreement with Capital
Financial Group, Inc., for provision of administrative services regarding the Town's healthcare
reimbursement account plan, and
WHEREAS, a proposed Administrative Contract and Service Agreement is presented at this
meeting,
NOW, THEREFORE, BE IT
RESOLVED, that the Queensbury Town Board hereby authorizes and approves the
Administrative Contract and Service Agreement with Capital Financial Group, Inc., for provision of
administrative services regarding the Town's health reimbursement account plan, substantially in the
form presented at this meeting, and
BE IT FURTHER,
RESOLVED, that the Town Board hereby authorizes and directs the Town Supervisor to
execute such Agreement, and
BE IT FURTHER,
RESOLVED, that the Town Board fiirther authorizes and directs the Town
Supervisor to execute firthire Agreements with Capital Financial Group, Inc.,
provided that there are no substantive changes to the Agreement, and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs that payment for services
shall be in accordance with the Agreement and shall be paid for from the proper account(s) as will be
designated by the Town Budget Officer, and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs the Town Supervisor and/or
Town Budget Officer to take such other and further action necessary to effectuate the terms of this
Resolution.
