During the course of construction I was on the job site to observe key components of the system
installation by Crandall Excavating. It should be noted that the system has been installed at the location
as indicated on the design plan although the inlet pipe into,,and discharge pipe from, the septic tank have
been slightly revised. This revision is now reflected in the attached`as-built' drawing. All other aspects
of the work were constructed as designed and using the materials as specified. Based on my inspection
of the system installation it is my opinion that the wastewater system is in compliance with.the intent of
the approved design plan and.is ready for operation.

application is as follows:

During the course of construction I was on the job site to observe key components of the system
installation by Crandall Excavating. It should be noted that the system has been installed at the location
as indicated on the design plan although the inlet pipe into, and discharge pipe from, the septic tank have
been slightly revised. This revision is now reflected in the attached`as-built' drawing. All other aspects
of the work were constructed as designed and using the materials as specified. Based on my inspection
of the system installation it is my opinion that the wastewater system is in compliance with the intent of
the approved design plan and is ready for operation.

GENERAL SANITARY NOTES:
1. MAINTAIN 100' SEPARATION BETWEEN SEPTIC SYSTEM AND ANY WELL.
CONTRACTOR SHALL VERIFY THE LOCATION OF ALL EXISTING WELLS
PRIOR TO CONSTRUCTION.
2. MINIMUM SEPARATION BETWEEN SEPTIC TANK AND BUILDINGS SHALL
BE 10 FEET.
J. MINIMUM SEPARATION BETWEEN EDGE OF BED AND BUILDINGS SHALL
BE 15 FEET.
4. MINIMUM SEPARATION BETWEEN PROPERTY LINE AND EDGE OF BED
OR TOE OF FILL (RAISED SYSTEMS) SHALL BE 10 FEET.
5. MINIMUM SEPARATION BETWEEN WELL AND SEPTIC TANK TO BE 50
FEET.
6. ALL CONSTRUCTION TO BE IN COMPLIANCE WITH THE NEW YORK
STATE SANITARY CODE.
7. REQUIRED INSPECTIONS (TO BE PERFORMED BY PROFESSIONAL
ENGINEER):
A) PRIOR TO PLACEMENT OF SEPTIC TANK OR COMMENCEMENT OF
THE ABSORPTION FIELD CONSTRUCTION.
B) DURING THE INSTALLATION OF ANY REQUIRED FILL MATERIAL.
C) PRIOR TO BACKFILLING BED.
D) FINAL INSPECTION UPON COMPLETION OF THE SYSTEM.
8. CONCRETE SAND SHALL BE A MEDIUM TO COARSE SAND WITH AN
EFFECTIVE SIZE OF 0.25 TO 2.0 MM AND NO MORE THAN 5% PASSING
A #200 SIEVE.
9. WHEN THE BED AREA HAS HAVE BEEN EXCAVATED, THE SIDES AND
BOTTOM SHALL BE RAKED TO SCARIFY ANY SMEARED SOIL SURFACES.

PROVIDE ACCESS FOR INSPECTION OF THE DISTRIBUTION BOX.
13. THE PIPES CONNECTING THE DISTRIBUTION BOX TO THE
DISTRIBUTION LINES SHALL BE TIGHT JOINT CONSTRUCTION LAID ON
UNDISTURBED EARTH OR PROPERLY BEDDED THROUGHOUT ITS
LENGTH.
14. THE SYSTEM DESIGN DOES NOT INCLUDE PROVISIONS FOR GARBAGE

ONE-THIRD THE HEIGHT OF THE FENCE.

E) THE SEDIMENT BASIN, IF PRESENT, WILL BE INSPECTED FOR DEPTH OF SEDIMENT, AND BUILT UP
SEDIMENT WILL BE REMOVED WHEN IT REACHES 50 PERCENT OF THE DESIGN CAPACITY.
F) TEMPORARY AND PERMANENT SEEDING AND ALL OTHER STABILIZATION MEASURES WILL BE INSPECTED FOR
BARE SPOTS, WASiOUTS, AND HEALTHY GROWTH.
G) A MAINTENANCE INSPECTION REPORT WILL BE MADE AFTER EACH INSPECTION. COPIES OF THE REPORT
FORMS TO BE COMPLETED BY THE INSPECTOR ARE INCLUDED IN THIS SWPPP.
H) THE JOB SITE SUPERINTENDENT WILL BE RESPONSIBLE FOR SELECTING AND TRAINING THE INDIVIDUALS
