In compliance with application approvals granted by the Town of Queensbury I offer the following
regarding the referenced project.
The design plan reviewed and approved by the Town as associated with the wastewater system
application is as follows:

During the course of construction I was on the job site to observe key components of the system
installation. It should be noted that based on the current condition of the fill sand(saturated gray/black
sand)installed during the original system installation in 1989 the Contractor excavated and removed
more than the 6”called for on the plan. Excavation reached the native sandy loam soil and
approximately two feet of'red dirt' was backfilled prior to installing the new Eljen absorption bed.
Based on my inspection of the system installation it is my opinion that the wastewater system is in
compliance with the intent of the approved design plan and is ready for operation.

WASTEWATER SYSTEM REMEDIATION
PROJECT DESCRIPTION: THE EXISTING ABSORPTION SYSTEM SERVING THE
BROTHERS' RESIDENCE WAS PERMITTED AND CONSTRUCTED IN THE FALL OF
1989. RECENTLY IT HAS EXHIBITED SIGNS OF FAILURE (BREAKOUT). IT IS THE
PURPOSE OF THIS PLAN TO PROPOSE A REMEDIATION TO THE ABSORPTION
SYSTEM BY REPLACING THE DISTRIBUTION FIELD WITH ADDITIONAL FILL AND AN
ELJEN GEOTEXTILE SAND FILTER (GSF) SYSTEM. REFER TO THIS PLAN SHEET
AND THE ATTACHED ORIGINAL DESIGN PLAN FROM THE 1989 PERMIT. HAND
WRITTEN NOTES HAVE BEEN ADDED TO THE ORIGINAL DESIGN PLAN TO CLARIFY
THE PROPOSED ACTION.
DESIGN BASIS: THE ORIGINAL DESIGN WAS BASED ON A FOUR BEDROOM FLOW
RATE WITH A TESTED PERC RATE OF 7 MIN./INCH. AS WE ARE PROPOSING
THE USE OF THE SAME BED AREA I AM RECOMMENDING THE USE OF ELJEN
GSF MODULES WHICH WILL PROVIDE ADDITIONAL TREATMENT AND CAPACITY.
FOUR LATERALS OF 28' EACH WILL PROVIDE 112 LF OF ELJEN GSF MODULES
ADEQUATE FOR 4 BEDROOMS 0 130 GALLONS/BEDROOM WITH A PERC RATE OF
15 MINUTES/INCH.
PROPOSED REMEDIATION: THIS PLAN PROPOSES THE FOLLOWING REMEDIATION
ACTIONS.
1.PROVIDE SILT BARRIERS DOWN -GRADIENT OF, AND EROSION
PROTECTION FOR, ALL AREAS OF LAND DISTURBANCE AND STOCKPILED SOIL
MATERIALS.
2. PUMP EXISTING SEPTIC TANK TO PROVIDE HOLDING CAPACITY DURING
CONSTRUCTION PERIOD.
3. STRIP THE EXISTING 8" - 12;'+ OF COVER MATERIAL OVER THE
EXISTING ABSORPTION BED AND STOCKPILE IT ON-SITE FOR FUTURE USE.
4. REMOVE, AND PROPERLY DISPOSE OF OFF-SITE, THE ENTIRE QUANTITY
OF THE EXISTING STONE ABSORPTION BED SYSTEM (FILTER FABRIC, STONE,
PVC PIPE AND CONCRETE D -BOX).
5. REMOVE 2" - 6" (AS NEEDED) OF THE EXISTING SOIL IMMEDIATELY
BELOW THE STONE BED TO REMOVE THE SUSPECTED "CLOGGED" SOIL
INTERFACE THROUGHOUT THE ENTIRE BED AREA.
6. PLOW OR TILL THE NEWLY EXPOSED SOIL TO A DEPTH OF 6" MINIMUM.
HEAVY EQUIPMENT SHALL BE KEPT OFF THE BED AREA TO PREVENT
COMPACTION OF THE SOILS SUPPORTING THE ABSORPTION FIELD.
7. ADD 12" OF SPECIFIED FILL MATERIAL (SAND TO LOAMY SAND) TO THE
PREPARED 'NATIVE SOIL' SURFACE. MECHANICALLY COMPACT THIS FILL
MATERIAL TO 90% STANDARD PROCTOR DENSITY.
8. ADD A MINIMUM 6" OF SPECIFIED CONCRETE SAND OVER THE SPECIFIED
BED AREA (20' X 32"±J. LEVEL THE CONCRETE SAND IN PREPARATION OF
ELJEN GSF INSTALLATION.
9. INSTALL A NEW CONCRETE D -BOX WITH BAFFLE. INSTALL SOLID 4"
PVC PIPING OUT FROM THE D -BOX TO THE LOCATION OF EACH LATERAL
RUN USING 45* BENDS AS NEEDED TO PROPERLY ALIGN TO THE LATERALS.
PROVIDE GRADE TO SOLID PIPE AS NEEDED FOR CONNECTION TO THE
PERFORATED LATERALS. SUPPLY SPEED LEVELERS FOR EACH LATERAL
PIPE.
10. INSTALL FOUR (4) LATERAL RUNS OF ELJEN GSF MODULES, EACH 28
LF IN LENGTH, IN COMPLIANCE WITH THE DETAILED CONFIGURATION.
INSTALL THE SPECIFIED 4" PERFORATED PVC PIPE AND WRAP WITH THE
SPECIFIED FILTER FABRIC PRIOR TO INITIAL BACKFILL WITH ADDITIONAL
CONCRETE SAND. COVER BED AREA WITH SUITABLE STOCKPILED SOIL,
SUPPLEMENTING WITH ADDITIONAL FILL SOIL AND/OR TOPSOIL AS NEEDED
TO COMPLETE TO FINISH GRADE.
11. ALL BED AREA INSTALLATION AS DESCRIBED UNDER 8-10 ABOVE
SHALL BE IN CONFORMANCE WITH THE MANUFACTURER'S (ELJEN)
SPECIFICATIONS AND RECOMMENDATIONS.
12. THE DISTURBED AREAS SHALL BE SEEDED AND MULCHED TO ESTABLISH
A STABILIZING VEGETATIVE COVER AS PER THE EROSION CONTROL NOTES.
13. SOME CONFIRMATION AND/OR REPAIR OF THE PUMP FLOAT AND
ALARM SYSTEM OPERATION IS EXPECTED TO BE REQUIRED OF THE
CONTRACTOR. DOSING VOLUMES WILL BE RE-ESTABLISHED FOR THE NEW
DISPERSAL SYSTEM BY THE ENGINEER.
14. THIS TASK LIST PROVIDES A GENERAL SUMMARY OF THE
CONTRACTOR'S REQUIRED ACTIONS. THIS LIST IS NOT INTENDED TO
COMPLETELY DESCRIBE THE WORK FOR WHICH THE CONTRACTOR IS
RESPONSIBLE.
15. WORK SHALL ALSO INCLUDE ANY APPLICABLE PERMIT CONDITIONS
SPECIFIED BY THE TOWN OF QUEENSBURY.

SYSTEM MAY BE UTILIZED, PROVIDED THE FOLLOWING SPECIFICATIONS ARE MET. FINAL APPROVAL OF FILL SYSTEMS SHALL
NOT BE GRANTED UNTIL THE FILL IS IN PLACE AND THE SYSTEM EVALUATED ACCORDING TO THE ADMINISTRATIVE
PROVISIONS OF THIS ORDINANCE.
B. THE DESIGN AND INSTALLATION OF A FILL SYSTEM SHALL COMPLY WITH THE FOLLOWING SPECIFICATIONS:
1) THERE MUST BE AT LEAST TWO (2) FEET OF NATURALLY OCCURRING SOIL (THREE (3) FEET WITHIN 1000 FT OF LAKE
GEORGE) OVER AN IMPERVIOUS LAYER.
